Payment must be made within one week prior to the first day of the class/course.  All cancellations or no show will be subject to a 10% cancellation fee with the exception of NREMT Testing.  The cancellation/no show fee for NREMT Testing is $50.00.

If paying by credit card the form can be filled out and faxed to 702-221-2155, emailed to training@medicwest.com or mailed.  Checks are to be written out to the location of the class (Medicwest Ambulance, AMR or NCTI).  All checks can be mailed or delivered in person to:
